fix conformance with 14496-15 AVCDecoderConfigurationRecord for HIGH profiles

Current code is based on old spec. Extra depth/chroma and required SPS extensions have been added for selected profiles.

SPS_EXT handling code has not been tested, as we do not seem to have any sample matching 0000012D or 0000016D NAL.
