Created Issue Template for Gitlab

I created Issue Template for Gitlab to help new users (and in general the people who open new bugs) to give us consistent info about what's happening and what's going on.

I took the examples (comments) directly from the old wiki page "trac.videolan.org".

Why did I create that new file: more info on the official doc

I also fixed a typo in the THANKS.md file
